Screening instruments · for rights holders

Third-party instruments are published only with confirmed permission.

The site offers health professionals a few validated screening instruments as on-screen forms that generate a PDF. This page explains, for authors and rights holders, how these instruments are handled and where each permission stands.

Publication rules

What we commit to for every instrument.

Permission first.

An instrument is published only when its license is confirmed in writing — public domain, an open license stated by the rights holder, or an approval letter. Instruments with restricted licenses or without express permission are not reproduced or adapted.

Faithful to the official version.

Items, response options, scoring and cut-offs are transcribed from the official Brazilian Portuguese version, unchanged. The authors, the copyright notice and a link to the source appear on screen and on the PDF.

Adaptations only where allowed, and flagged.

Where the rights holder permits it, the only local adaptation is practical — for example, replacing crisis resources from another country with Brazilian ones (CVV 188, SAMU 192). Any such change is identified as an adaptation.

Stated limitations.

Where cut-offs were validated in another population (for example, adults), the form says so next to the score, on screen and on the PDF.

Free and non-commercial.

No fees, no advertising, no login, no sponsorship. The instruments are offered to public health and juvenile justice professionals as an aid to — never a replacement for — clinical assessment.

Removal on request.

If a rights holder asks, the instrument is taken down promptly.

How the forms work

Everything happens in the professional’s own browser.

The form is filled in on screen and the PDF is generated locally by the browser. Nothing is sent to a server and nothing is stored — neither on this site nor on the user’s computer.

A blank PDF is also available for printing. The site is static, uses no cookies or trackers, and loads no third-party resources.

The completed PDF belongs to the professional, to be printed, signed or attached to the official health record.
